Job Title: Advanced Practice Nurse – Mobile Wound Care | Paid Certification Provided

Company: Twin Cities Physicians

Compensation: $105,000 to $120,000 annually

Benefits: Full Benefits Package (for full-time status)

Location: Greater Twin Cities Metro Area, MN

Schedule: Full-Time

Position Summary:
This is a field-based role for a Certified Nurse Practitioner (CNP or AGNP) interested in wound care and long-term primary care. No certification in wound care? No problem. We’ll help you get there—and cover the cost.

Key Responsibilities:

• Visit multiple senior care sites each week
  
• Deliver in-place wound evaluations and follow-up care
  
• Participate in care coordination and medication reconciliation
  
• Build trusted relationships with residents, families, and facility teams
  

Qualifications:

• Current MN CNP license
  
• Wound care certification or willingness to obtain
  
• Background in long-term care or family/geriatric practice
  
• Must have reliable transportation
