# Physical Therapist Assistant (PTA) — Home Health (Rockford, IL)

You became a PTA to help people move, recover, and live more fully. At Abcor Home Health, you’ll do exactly that — with the support of a family‑owned agency that values you and the patients you serve.
We’re hiring a Home Health PTA to provide hands‑on care across Rockford and nearby areas, under the supervision of a licensed PT.
## What you’ll do
- Implement the PT’s plan of care with individualized, in‑home treatments
- Guide patients through therapeutic exercises, gait/balance work, functional training, and home safety strategies
- Teach and coach patients and caregivers on home exercise programs and equipment use
- Track progress, report changes, and collaborate closely with the supervising PT and the care team
- Document visits and outcomes in the EMR clearly and on time
## What we’re looking for
- Active Illinois PTA license; accredited PTA program graduate
- Home Health or related rehab experience preferred
- Strong patient communication and teaching skills
- Ability to work independently in diverse home environments; reliable documentation habits
## Schedule & training
- Minimum 5 visits per week
- Flexible scheduling you design
- Paid, tailored training (~2 weeks) in Arlington Heights
