We are seeking a dedicated and experienced Registered Nurse to join our team as a Patient Care Manager in Home Health . In this pivotal role, you will oversee and coordinate the delivery of comprehensive nursing services to patients in their homes, ensuring the highest standard of care is consistently provided. Key Responsibilities:

• Manage and direct the day-to-day clinical operations of the home health team to ensure quality and compliance with all regulatory requirements.
• Conduct comprehensive patient assessments and develop individualized care plans in collaboration with the interdisciplinary team.
• Supervise and support home health staff, ensuring the delivery of exceptional patient care services.
• Facilitate effective communication between patients, families, and healthcare providers to ensure coordinated and efficient care delivery.
• Monitor patient progress and modify care plans as necessary to achieve optimal health outcomes.
• Participate in quality improvement initiatives and ensure adherence to best practices in home health care.

Qualifications:

• Current Registered Nurse (RN) license in the state of GA.
• Minimum of 1 year of nursing experience, preferably in a home health or community setting.
• Home Health leadership experience required.
• Demonstrated leadership skills and experience in managing healthcare teams.
• Strong knowledge of home health care regulations and standards.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage multiple priorities effectively.
